Open & Switch Enrollment 2018

Thursday, Oct. 18 through Friday, Nov. 30

The following changes can be made during this open/switch enrollment period:

• MEDICAL
o Employees can switch medical plans (high option to low option or low option to high option and/or Blue Preferred Exclusive Provider
Organization (EPO)
o Employees can switch medical carriers (BCBSNM to Presbyterian or Presbyterian to BCBSNM or Blue Preferred (EPO)
o Employees who previously declined medical coverage or missed the deadline for enrolling, can enroll to medical coverage (including applying
to enroll eligible dependents)
o Employees can decline medical

• DENTAL
o Employees can switch dental plans (high option to low option or low option to high option)
o Employees who previously declined dental coverage or missed the deadline to enroll, can enroll to dental coverage (including applying to
enroll eligible dependents)
o Employees can decline dental

• VISION
o Employees who previously declined vision coverage or missed the deadline for enrolling, can enroll to vision coverage (including applying to
enroll their eligible dependents)
o Employees can decline vision if they had this benefit for at least 2 years

If you like to make any changes please stop by to fill out a change cards with Alicia Huerta between 9 a.m. and 4 p.m. Thursday, Oct. 18 in Jemez Rooms during the Health Fair and after that date in room 309A.

Eligible employees: Employees working 30 hours or more are eligible for medical insurance, regardless of status.

*Deadline to submit a Change Card form is Friday, Nov. 30, 2018.
