The Make-up and Accommodations Form is available for individual students who cannot be present for a test in a campus class. Accommodations testing is reserved for individual students who have received an accommodation through Student Accessibility Services. The form needs to be completed by the instructor.
The Testing Center staff administers the test according to the directions included within the form. Students are asked to reserve a seat for the desired test date on The Testing Center Bookings site located on MYSFCC. The appointment can be made within the “Schedule an Appointment” link.
