Treatment Records

According to FERPA, a medical record may be considered a treatment record and is not considered an education record. Unless noted otherwise by HIPPA, a treatment record must be: a. Directly related to a student who is 14 years or older and/or who is attending SFCC; b. Made or maintained by a physician, psychiatrist, psychologist or other recognized professional such as an SFCC licensed mental health counselor or paraprofessional acting in a professional capacity or assisting in a paraprofessional capacity; c. Made, maintained or used only in connection with the provision of treatment to the student and not available to anyone (including the student) other than persons providing such treatment (such as an SFCC licensed mental health counselor), except that such records can be personally reviewed by a physician or other appropriate professional of the student’s choice.
